The Nitto Dirtdrop was originally designed to make drop bars work on too-small, too-longa-top tube mountain bikes, and was original equipment on the 1987 Bridgestone MB-1. Its inspiration was the original Cunningham LD stem.

History aside, it's still our top choice for that kind of use and for most bikes with Moustache Handlebars, since they work best with a tall and short stem. You can get them there with a Technomic Deluxe stem, which has a more or less horizontal extension and therefore looks more normal; but this one gets them even higher, or: Just As High Widdout Exposing So Much Quill.  For a big guy on a Moustache H'barred bike, we'd recommend this in a second.

Two extensions. The 80 (wood stump pic) has an effect horizontal extension of 65mm; the 100's (graph paper pic) is probably around 82 or so. The up-angle makes the difference. 22.2mm quill, so it fits all of our bikes; 26mm clamp, so it fits all the bars we stock except the Albatross and Dove. Cold-forged and heat-treated aluminum. Super strong.

Get the Nitto 26.0 to 25.4 shim (part #16-095) if you're gonna use the Albatross or Dove.  Or make life easier and less expensive and get the Periscopa stem which has a 25.4mm clamp. The Periscopa is not Pope-blessed for hard off-road use, though.
